Eligible Applicants: 

  • Located in Minnesota

  • For-profit or not-for-profit entities that demonstrate extensive experience providing in-depth, high-quality education and training and/or developing resources that support rural health care.

  • Be trained in iECHO, for the Project ECHO model.

  • Applicants will be selected based on their ability to complete proposed projects and evidence of experience in the proposed subject matter. 

The Rural Health Transformation Program Activities are supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $193,090,618.14 with 100 percent funded by CMS/HHS.
