Grant to Prevent Sleep-Related Infant Deaths in Minnesota’s American Indian and Black/African American Populations

The Minnesota Department of Health (MDH) seeks proposals to fund one organization with the capacity and expertise to create, manage, and support Community Action Teams (CATs) to implement culturally appropriate, data driven solutions to reduce sudden unexpected infant deaths (SUIDs) in Minnesota’s American Indian and Black/African American populations. The organization selected for this grant must be uniquely positioned to support the CATs in implementing community prioritized solutions to improve infant sleep safety that align with the American Academy of Pediatrics safe sleep recommendations. The awarded applicant must also be immediately ready, upon being notified about the award, to begin forming the CATs and executing other duties as contractually agreed upon. This RFP aligns with both state and national goals to reduce infant mortality by addressing SUIDs in communities that are disproportionately affected by sleep-related tragedies during infancy.

The source of funding for this grant opportunity is the Centers for Disease Control and Prevention, Department of Health and Human Services, through NU58DP007698. Funding is currently available for $134,000 from July 15, 2026, to Sept. 29, 2027. However, the grant may be extended for an additional year contingent on the availability of funds and the awarded applicant’s performance during the initial project period. Funding will be allocated through a competitive process.

Eligible applicants include, but are not limited to:

  • Community health boards (CHBs) / local public health

  • Nonprofit organizations

  • Tribal governments

  • Clinics

  • Hospitals

  • Institutions of higher learning

Application deadline: Completed applications must be submitted to the grant interface portal, Foundant, by 4:30 p.m. on Friday, May 22. Late or incomplete applications will not be accepted. MDH is not responsible for technological issues that prevent the application from being submitted on time.

    2. Located outside of the seven-county metropolitan area and serve rural communities. The seven-county metropolitan area includes Hennepin, Ramsey, Dakota, Anoka, Washington, Carver, and Scott counties. 

  • Applicants may include:

    1. Established Child and Adolescent Mental Health Collaboratives (including Integrated Children’s Mental Health and Family Services Collaboratives)

    2. Counties or consortia of counties/county health boards

      1. Current consortia/regions serving Minnesota’s Adult Mental Health Initiatives are strongly encouraged to apply in the same configuration

    3. Tribal Nations

  • Telehealth providers that are currently enrolled as a MHCP provider or demonstrate a plan and timeline to achieve MHCP provider enrollment

  • Partnerships or consortia of the above entities (one entity must serve as the fiscal lead)

MDH 2026 Workplace Safety Grants for Health Care Entities

The 2026 Workplace Safety Grants for Health Care Entities Program Grant application will open on Monday, April 20, 2026. All applications must be submitted through the ORHPC Online Grants Portal by 4:30 p.m. Central Time on Friday, June 12, 2026.

The Workplace Safety Grants for Health Care Entities program, authorized by 2023 Session Law Chapter 70, Article 4, Section 109, is established to award grants to increase safety measures in health care settings and establish or expand programs to train staff in health care settings on de-escalation and positive support services. The program is focused on preventing workplace violence. The overarching goal of the Workplace Safety Grants for Health Care Entities program is to make long-term improvements in safety and stability for staff and patients in health care settings.

The following health care entities are eligible to apply for funding:

  • Long-term care facilities

  • Acute care hospitals that are staffed for 49 beds or fewer and located in a rural area

  • Critical access hospitals

  • Medical clinics

  • Dental clinics

  • Community health clinics
     

Health care entities with multiple locations may submit a maximum of three separate applications for distinct projects happening at different locations or one application covering work that spans multiple locations. The award for each application will not exceed the statutory limit of $50,000, even if an application includes work at multiple locations.

MDH anticipates awarding approximately $1,000,000 t0 $1,250,000 through 20 grants in this final funding cycle. The maximum award amount is $50,000.
