We are excited to be hosting the Gillette Children's Pediatric to Adult Health Care Learning Collaborative Summit, which is the culmination of the eight-month, virtual learning series.

This conference will be held Thursday, June 13, 8:15AM – 4:45PM, at the Holiday Inn & Suites in St. Cloud, Minnesota.

 Learning Objectives: Participants attending the Transition Summit will be able to:

  • Describe the value and need for youth and family engagement during health care transition.

  • Analyze evidence-based resources and apply tangible skills in the implementation of health care transition processes within your own organization.

  • Employ trauma-informed care and health equity principles into health care transition processes.

  • Gain insights from other organizations, nationally and from Minnesota, who initiated implementation of the Six Core Elements of Healthcare Transition.
