Many Faces:The Call for Presentations will open on April 8th!

We are seeking presenters experienced in working with populations from diverse communities that face significant health, social, and economic challenges. We're looking for abstracts for 60-minute oral presentations describing your practical, tested, culturally-appropriate strategies or promising practices that can be implemented in clinical or community settings, and address any of the following: 

  • Advancing health equity and addressing social determinants of health

  • Addressing the needs of patients with, or at risk, for chronic diseases

  • Addressing quality of care, care redesign or operational improvements, team-based care, or care coordination and primary care integration (including medical, behavioral, and/or oral health)

  • Addressing the unique challenges facing safety net providers

Submissions due by May 20th at 11:59 PM.
